Free and Reduced Lunch Application

The Free and Reduced Lunch Application form is available in two convenient formats. The Microsoft Word Template format allows you to fill out the form on the computer and then print. The Adobe Acrobat (PDF) file can be printed and then filled out by hand. Either version must then be mailed or brought to:

Food Allergy Form

You must fill out a new form EACH school year if your child requires substitutions to his/her school lunches due to allergies or intolerances. The form must be signed by a recognized medical authority to be accepted. Return to your school office or send to:
