Below is the 2025-2026 Consent for Disclosure Form (Enrollment Fee Waiver) and printable Free & Reduced Meal Applications.

If your family qualifies for Free or Reduced-Price School Meals, you may complete the Consent for Disclosure Form to apply to have enrollment fees waived. 

If you are unable to print these forms, you can pick them up at any of the Goddard Public School buildings, or we can mail them to you.
